During kindergarten registration, parents and guardians must provide an official birth certificate (not a copy); a copy of the immunization record; custody papers in case of a divorce or legal separation (decree if applicable); photo identification (driver’s license, state ID card or passport); and proof of residency, such as a current utility bill or rent receipt.
